The Non - Supervisory Merit employee plan year for medical insurance goes from January 1st to December 31st. The State of Iowa holds an open change period annually. Information is sent to employees each October outlining the changes that are allowed during that year's open change period.

Wellmark Blue Cross/Blue Shield Plan 3 Plus is an indemnity plan. You may seek treatment from any licensed physician anywhere in the world.

Benefits will be based on prevailing or usual, reasonable and customary (URC) fees for the service area in which the physician practices. If the physician charges more than URC, and is not a Blue Cross/Blue Shield participating physician, you will be liable for the excess charges.

Iowa Select is a preferred provider organization (PPO). If you utilize a participating physician and/or participating hospital, you will be covered by one set of deductibles and co-insurance. If you utilize a non participating physician or hospital, you will be covered by a different level of benefits.
